DEFINITION
Process recording is the interaction or interview conducted and recorded by the nurse by using various communication techniques
PURPOSES
- To study the patients psychological, social and emotional behavior
- To evaluate the conduction of patient during admission, hospital stay and at the time of termination of hospital stay
- To improve therapeutic communication skills of the nurse
- To establish a therapeutic relationship
GENERAL GUIDELINES
- Record the conversation verbatim
- Use a recording device and obtain patient’s permission for using it. This will help in reviewing the session if needed
- Note the non-verbal responses of patient and nurse when recording the session
